State Specific Contract Provisions
If required by your state, certain provisions are included in your contract, as set out in the State Law Coordinating Provision (SLCP) exhibit. Periodically, we make modifications to the SLCP exhibit to reflect changes in state law. For a copy of the current SLCP exhibit, please click on the appropriate State listed below.
These contract provisions are not applications to participate in our networks. To apply for participation, click the Join our Network button to the left and complete the required fields. Note that if you are an individually contracted practitioner in either the MultiPlan or PHCS networks, you are automatically included in both networks.
